How referrals work

To refer a patient to Aptiva, gather four things — the patient’s demographics, insurance/payor information, the relevant records explaining the reason for referral, and any imaging reports and images — complete the referral form, and send it by secure email or fax. There’s no online form that collects patient information here; referrals are sent through secure channels to protect privacy.

Step 1 — Gather the information

Have ready:

Demographic data

for the patient.

A copy of the insurance card or third-party payor information

Relevant medical records

that explain the reason for the referral.

Imaging reports and the associated images

, if any.

Step 2 — Complete the referral form

Download and fill out the New Patient Referral Form — a fillable PDF you can complete on your computer or print and fill in by hand.

Step 3 — Send it securely

Send the completed form and attachments by secure email to referrals@aptivahealth.com, or by fax to 1-855-859-0123. Use a secure/encrypted method — please don’t send patient information through unsecured email.

What happens next

Aptiva’s team reviews the referral, contacts your patient to schedule — often quickly — and coordinates any imaging or specialist visits internally. When the episode of care is complete, records are returned to your office.

Is there an online referral form?

We intentionally route referrals through secure email or fax rather than a public web form, to keep patient information protected. Download the form, then send it securely.

Do I need to call first?

No, though you’re welcome to. Questions: aptivahealth.com/refer-a-patient.

What if I don’t have imaging to send?

Send what you have. Aptiva can perform needed imaging on site.

How will I know my patient was seen?

Records come back to your office after the episode of care; see our records commitment.
