Aptiva Health is committed to returning records to referring providers within 3 business days, so your patient’s care isn’t held up waiting on paperwork. This page explains how to request records and what to expect.
Why timely records matter
A referral isn’t really complete until the information comes back. Slow records mean delayed follow-up, repeated tests, and frustrated patients. Aptiva treats the return of records as part of the care, not an afterthought — so you can close the loop and keep caring for your patient.
How to request records
Request records through the records portal → the records portal, or contact the records team at dbrown@aptivahealth.com or 1-844-999-3627. For imaging specifically, you can get direct cloud access (see Imaging portal) rather than waiting on a copy.
